My child has my permission to attend a Texas QB event during 2010. I certify that I am the parent or legal guardian of the player registering for the Texas QB event. I certify that my child has been examined by a licensed physician within the last 12 months, and is able to participate in all football program related physical activities. I understand that my child may come in contact with other people & objects, and I hereby waive all rights to future claims related to such contact or injuries sustained as a result of such participation or contact. I agree to assume any & all risks associated with my child's participation in a Texas QB event and release the host site, any & all representatives or members of host site, Texas QB, Jason Lovvorn, Erick Franklin, & any & all assistants of Jason Lovvorn from any & all liability related to injury or illness my child may sustain at a Texas QB event.
